We want to share an important update from the Utah Department of Corrections (UDC) regarding the process for securing housing for formerly incarcerated individuals. Recently, UDC has been experiencing some technical difficulties with the online system (Wufoo) used to submit housing request forms (found at (UDC Website > Probation / Parole Menu > Address Verification Form). As a result, the form is currently offline and not available on their website.
While the UDC team is actively working to resolve this issue, families and friends who are assisting their loved ones with housing applications may need to complete the form manually. The form can be printed, filled out, and then emailed to Jodi Huyboom at [email protected]. From there, Jodi will ensure the information is passed along to the appropriate case managers for follow-up.

2. Complete the necessary information.
3. Email the completed form to [email protected].
We understand this temporary manual process may be inconvenient, but UDC is working diligently to restore the online system as soon as possible. In the meantime, your support and understanding are greatly appreciated.
